• Visit the Academic Affairs Division of the university to obtain the form.

Application Fee

• The application fee is ₦10,000.

Registration Requirement

• Applicants must have completed registration for the 2024/2025 Academic Session before applying.

• The deadline to obtain the form is 14th February 2025.
